Objectives :
Identify and eliminate waste through effective Root Cause Analysis
This course is designed to teach the attendees how to use process driven root cause analysis, and includes some of the most popular and useful problem solving techniques available. The attendee will leave this workshop solving problems faster and with more confidence in their solutions. These objectives are accomplished with a simple set of learning aids that can be used each time Root Cause Analysis (RCA) is applied in your facility.

- Change Analysis;
- 5 Whys;
- Fault Tree;
- Logic Tree;
- Sequence of Events;
- Event and CausalFactor Analysis;
- Simplified Failure Modes Analysis (FMA); and
- Failure Modes, Effects, and Criticality Analysis (FMECA).
Class lecture will be reinforced with numerous practice opportunities, so the attendee will be prepared to go back to the job and attack chronic plant problems and incidents right away. During the in-class time, you will learn to :
- Outline the steps of a successful root cause process;
- Apply tools like Failure Reporting, Analysis, and Corrective Action System (FRACAS) to identify reoccurring issues;
- Define which events should trigger a root cause investigation;
- Describe when to use each RCA tool;
- Build business case thinking into every step of your RCA process
- Recall how to ensure success and sustainable solutions.
